What TheraNest does

TheraNest is an electronic health record and practice management system for mental and behavioural health practices. It covers scheduling, progress notes, treatment plans, a client portal, telehealth, credit card processing and insurance claim submission, and it is sold per therapist rather than per practice. What distinguishes it commercially is that insurance billing is the tier boundary. The entry plan carries no claims at all, the middle plan includes a fixed monthly allowance of claims and the top plan removes the cap. A practice that bills insurance heavily is therefore not choosing between feature sets so much as choosing how much of its revenue cycle it wants to pay a per claim toll on. There is also an annual AMA CPT licence fee charged separately each December, which is a real line item most comparison articles leave out. Buyers are typically solo therapists, small group practices and community mental health organisations that want claims and notes in one system without a consultant led implementation. The trade-off is a product with a long history and an interface that shows it, inside a company that has been consolidating brands, so the roadmap is set by portfolio strategy rather than by this product alone.

Where it falls short

Concrete and checkable, so you can decide whether any of them matter to you. This is the half of a review a vendor will not write about TheraNest.

  • The Essentials plan includes no insurance claims at all, so any practice that bills a payer is effectively on the 59 USD tier or above and the advertised 29 USD entry price does not apply to it.
  • Telehealth is only in the Premier plan, so a practice that wants video sessions pays 89 USD per therapist per month even if it needs nothing else from that tier.
  • An annual AMA CPT licence fee is billed separately each December on top of subscription cost, which surprises practices that budgeted only the monthly figure.
  • Additional administrative users are chargeable, so a practice with front desk and billing staff who never see clients still pays 19 to 29 USD per head for people who generate no revenue.
  • The product sits inside a consolidated portfolio that has already renamed the parent company from Therapy Brands to Ensora Health, and product investment in a portfolio is directed by the owner rather than by this product line, so roadmap commitments are worth less than they would be from a single product vendor.
