The app is. Using it is not, unless your surveys are public. Authoring needs a Creator licence and each person completing a secure survey needs a Mobile Worker, Editor, Creator or GIS Professional user type.

Yes. It is MIT licensed, which permits commercial use and embedding without a copyleft obligation.
